If you are current or former client of Watershed Wellness, PLLC you can request your records in writing by emailing melissa@watershed-wellness.org or by mailing a signed letter to 1550 Norwood Drive Suite 120 Hurst, TX 76054. Please include your name, birthdate, a picture of your driver's license, the reason for needing records, and how you wish to receive records. I will review your request, determine what will be released, and process payment of the records request fee outlined in the PHI consent and your informed consent, and respond to you in writing or by phone to verify your request.
Fees and guidelines per the informed consent are below:
A client or their guardian has a right to review their mental health record. If the counselor determines that viewing some of the information will be detrimental to the client’s mental health, the counselor may offer a treatment summary or suggest having a third party view the information to help determine if it is detrimental the client’s mental health. Requests to view or receive copies of records should be made in writing and per the LPC board rules the counselor must respond within 15 business days. Please give the counselor at least 7 days notice for records requests.
In accordance with state standards, Watershed Wellness, PLLC will follow the following fee schedule for copies of mental health records. These fees are to be paid by you before the records are released unless there is an emergency situation:
• A basic retrieval and processing fee of $30, which includes the cost of the first 10 pages copied if paper copies are requested.
• Pages 11 to 400 will be .25 cents per page.
• The actual cost of mailing, shipping, or delivering the copies.
• Per state law, no fee will be charged for billing records or the first copy of mental health records that are requested for disability purposes.
You can always discuss concerns with the counselor. If you have a complaint regarding a violation of ethics or professional conduct this should be filed with the Texas Behavioral Health Council. All complaints must be submitted on the Council-approved complaint form. The complaint form shall be obtained free of charge from the Council’s website (https://www.bhec.texas.gov/discipline-and-complaints/index.html) or by requesting a copy from the council.
